When making a written report of abuse, what should you include in your report?

  • Anything that the child discloses
  • Questions you have asked and answers given (in first person)
  • Signs and symptoms of suspected abuse
  • Size and location of injuries
  • Date and time of the disclosure
  • Date and time of the incident
  • Describe caregiver behaviour that is of concern
  • Describe any behaviour of the child that is of concern
  • The type of risk the child may be facing
  • As many details as possible about what you have witnessed
